If you completed registration before the 2024-2025 school year ended, your child(ren) are already registered. New families have received an email with instructions about how to register online. • Health exam: Illinois law requires a health examination for students entering Pre-K, Kindergarten, & 6th grades and all students entering or transferring into an Illinois school for the first time. https://5il.co/t4dg

• Eye exam: All children enrolling in kindergarten or upon first entry into an Illinois school beyond kindergarten are required to have an eye examination. Examinations must be performed by a licensed optometrist he/she shall complete and sign the Eye Examination Report form. These examinations should be completed before the start of school. https://5il.co/t4de

• Dental exam: All Illinois students in kindergarten and Grades 2 and 6 are required to have an oral health examination. https://5il.co/t4df

• A "Live Birth Certificate" copy must also be submitted for all the Pre-K and Kindergarten students as well as any student new to the district.

• School Medication Authorization Form: All prescription and non-prescription (over-the-counter) medications require written authorization from the student’s health care provider (licensed to prescribe the medication) as well as written parental consent. A medication authorization form can be obtained from the office.
